I understand and agree that all photographic and video images submitted or produced as a result of my participation in the Joe Weider 2000 Model Quest, The making of the Weider Girl and Guy Model Quest video, Wild on the Riviera at the Bettie Davis Estate in Laguna Beach California, Making of the Weider Girl and Guy Calendar, and the modeling for print in the magazines, clothing, or products T.V. show which are the exclusive and sole property of Mike Glass Inc. / president of the 2000 Weider Model Quest Inc. for its universal usage. I understand and agree that I have no interest in any photographs, or their usage that are generated or submitted as a result of my participation in the aforementioned event.

I also understand that these events will be videotaped for possible usage on T.V., syndication and home viewing. By signing my name below, I  grant the producer, employees, agents and assigns the following at no cost: the right to photograph me and use my picture, silhouette, and other reproductions of my physical likeness (as the same may appear in ant still camera photography and/or motion picture, films or tapes) and in connection with the exhibition, theatrically, on television and otherwise, of any television show home video cassette, internet, motion picture print posters, magazines, calendar, but not limited to\o the above. I father give the producer the right to reproduce, in any manner whatsoever, any recordation of my voice and all instrumental, musical or other sound effect produced by me. 

 I hold Joe Weider's 2000 Model Quest, Mike Glass Productions, Mike Glass President, its assigns, sponsors, and associates, including but not limited to, Joe Weider's 2000 Model Quest, Mike Glass Productions, Weider Health and Fitness Inc., The making of the Weider Girl and Guy Model Quest video, Wild on the Riviera at the Bettie Davis Estate in Laguna Beach California, Making of the Weider Girl and Guy. All our sponsors, all production crew, the Bettie Davis Estate and their owners, exhibitors, stage crew, make-up artist, judges, Talent agents, master of ceremonies,   Weider Health and Fitness Inc., Photographer Per Bernal, Dr. Richard M. Hodnett, Dr. M.H. Patel D.D.S.,P.C. Gold's Gyms Inc., Las Vegas Athletic Clubs, and Isuzu the official Vehicle blameless and without liability in connection with my participation in Joe Weider's 2000 Model Quest and related events. I do assume all responsibility for myself, my behavior, and my well being for the duration of and traveling to and from any of the Joe Weider's 2000 Model Quest, Joe Weider's 2000 Weider Awards, Mike Glass Productions, the Bettie Davis Estate of Laguna Beach, the Four Seasons Hotel of Las Vegas, the Mandalay Hotel and any and all participation of the above events. These Awards are not affiliated with Weider Health and Fitness Inc.
